To do so, a case manager is assigned to coordinate care with your physician or primary caretaker. The case manager creates a customized plan of care, assigning team members who have been specifically trained to meet the needs of your case. Whether it is companionship, support with daily activities, or even cooking, Comfort Home Care Agency has the expertise to deal with even the most complex of cases.
